HEALTH ASSESSMENT

The Cochiti Community Health Assessment survey is now available for ALL Cochiti Pueblo residents  to complete! The survey can be accessed via QR code on the flyers or here . For log-in information to access the survey online, please contact Antoinette Bird or Cameron Quintana, their contact info can be found on the flyer. Paper surveys can be requested as well. Please contact Antoinette or Cameron for further information or questions. Those who complete the CHA survey in is entirety will be eligible to receive a $75 gift card.
